Editing Disclosure Templates
Last updated: October 17, 2026
Disclosure templates define the questions employees answer when they file a disclosure, such as an outside business activity disclosure. This article explains how to open a template in the template library, add, change and remove questions, and publish your edits. Do this when your firm's disclosure requirements change and you need to capture different information.
Open the template
- Click the gear icon at the top left to open Settings, then go to the template library. The library is where every reusable form across your workflows is managed.
- Browse the disclosure templates and open the one you want to update.
The template opens in a read-only view. It shows exactly what employees see when they file a disclosure, so it's a good place to review the existing questions before you change anything.
Switch to edit mode
To make changes, switch the template into edit mode. Your changes are not visible to employees until you publish them.
Add a question
Add a new question and set the following options for it.
- Question type: The type controls how employees respond. A short answer suits free text, such as a location. A date question gives you a clean, consistent date, such as a start date. Multiple choice lets employees pick from a list of options.
- Question prompt: Write the question as you want employees to see it. For a date question, label it so employees know which date you need.
- Required: When a question is required, employees can't submit the disclosure without answering it.
- Context: Add guidance so respondents know exactly what you're asking.
- Flags: Flags notify compliance automatically when an answer meets conditions you define.
Flags are the best way to make sure compliance sees the answers that matter without reviewing every submission by hand.
Edit or remove existing questions
You can change any existing question. For example, a yes or no question can be switched to multiple choice so employees can give more detailed answers. When you change a question's type, the platform warns that existing settings may reset. Confirm the change if you want to continue, then add the options you need. You can add more options at any time.
To remove a question you no longer need, delete it from the template while you're in edit mode.
Publish your changes
When your edits are complete, click "Publish" to make this version live. The platform asks you to confirm, and once you do, the updated template is ready for new disclosures.
What happens next
After publishing, you return to the template library. Every new disclosure uses the updated questions. If you need to make further changes, open the template and switch to edit mode again.
